MEDICAL FORMS & INFORMATION

Medication Policy: A completed Medication Authorization form needs to be completed by your child's medical provider for any prescription and non-prescription medication to be given in school.  

 

Parents must deliver all medications directly to the school nurse for the safety of all students.  It is also the responsibility of the parent to pick up any remaining medication at the end of the school year.  Medications will not be kept over the summer, they will be discarded.  Each school year a new medication authorization form must be completed.

Health Information:  Please notify the nurse if your child has any health problems or is receiving follow-up care by a physician.  The nurse will update the health record and will be able to better evaluate your child if he/she has any problems.  It is also helpful if the health office is made aware of any medications the child is taking at home.

Food Allergies: Please be aware that some students have food allergies which can be life threatening.  It is important to check with your student's teacher and/or the nurse prior to sending in any snacks being served in the classroom.

Illnesses:  A student that has a fever of 100.0 or higher will be excluded from school and may not return until they are fever free for 24 hours WITHOUT any fever reducing medication in their system.  

 

Injury: If a student has obtained an injury that requires them to be out of physical education - a doctor's note is needed.  If the doctor's note says the student will be excluded until further notice, then the student will need another doctor's note that states the student has no restrictions to be able to return to physical activity.
